Forms:Plans of Study, Independent Study, Residency, Dissertation-related Forms, Certification, and Graduation Each program has its unique plan of study. Requirements frequently change. Students must work with an advisor and have a signed plan by the student, advisor, Department Chair, and Graduate School Dean to ensure that courses in the plan will "count" towards certification or degrees. Unsigned plans are not binding on the part of the Department. If students use plans that are not signed to self advise, they risk having to take additional courses. Residency - Complete Form before beginning Residency
Doctoral students must enroll for two semesters of residency by completing the residency form before they graduate. This requires a minimum number of course hours completed within an 18-month period. Below shows the minimum hours for each semester. In the example below one could declare residency any two consecutive semesters or take 6 hours during Summer 1 and Summer 2. If one declares a fall or spring semester as a residency semester, one must enroll for 9 hours. One may use dissertation (599) hours for residency. One may not RETROACTIVELY declare previous semesters, so please complete the form in a timely manner.
