Self-Placement Observations

Instructions

Before observations are made, the self-placement form should be completed by the student and the principal of the school in which the student will observe. For convenience, the form may be faxed to the principal for completion. To prepare for faxing, the student should complete student, semester, and course information. Additionally, the number of clinical hours needed (in the line that begins, "Yes, this student is allowed to complete ___ clinical experience ....") should be completed by the student before faxing. The principal should complete school/agency/site and administrator information. On the line before "Yes, this student is allowed ... ", the principal should place an "X" or a check mark.

The self-placement form is valid for the entire school year. If the principal is in agreement, the student may observe multiple teachers within that school. Because the teacher's driver's license number is required, please do not leave this line blank. If the teacher hesitates to provide her/his driver's license number, the teacher's birth date is acceptable.

Remember, the self-placement form must be completed before any observations are made. After the observations are completed, please return the form to the ISU course instructor.
